  • We accept most major insurance plans and are happy to help you understand your benefits. We also participate in the Canada Dental Care Plan (CDCP) to help eligible patients access affordable dental care.

  • Please bring a list of any current medications, your insurance information, and a completed new patient form (which can be downloaded from our website). If you don’t have the form ahead of time, we’ll have one ready for you at the clinic — just arrive 15 minutes early.
